purpose of this test is to detect any NEW changes in your vision.

| 1. |
Please
print and then place Amsler Grid in a well lighted area at a comfortable
reading distance (14 inches). |
| 2. |
Wearing
your Reading Glasses (if you wear glasses) check each eye individually. |
| 3.

Stare
at center dot and ask yourself the following questions:

A. |
Can
you see the dot in the center? |
|
B. |
Are
there any blank or dark spots within the grid? |
|
C. |
Is
there ant new distortion or waviness of lines? |
|
D. |
Are all four corners present? |
|
|
|
| If
